When the 17-year-old boy says he plays baseball and would like to get an athletic scholarship for college, we can suspect that he spends a lot of time in the field, exposed his self to sun and has been exposed to smokeless tobacco which is prevalent in the baseball sport. An increased use of smokeless tobacco use in sports, mainly in baseball is been reported  in 2016 (Jarvis, 2016). ), “Leukoplakia is chalky white, thick raised patch with well-defined borders.”  This is attached firmly to the buccal mucosa, even if you scraping hard it will not come off. You can also see it in individual who are heavily tobacco user as well as alcohol (Jarvis, 2016). White patches in the mouth also called excessive Candida you may see it in patient with weak immune system, steroid inhaler user, and HIV infection, use of broad-spectrum antibiotics, leukemia, and malnutrition.Slight upward or downward slanting of palpebral fissures normally occurs on a familial basis or in groups such as people of Asian descent. So in this case the slating palpable fissures in his eyes found during the assessment can be suspect that this patient is Asian or is an Asian descent. In non-Asians, the palpebral fissures are horizontal (Jarvis, 2008). Usually when narrowed, upward slanted palpebral fissures are present it could be indication of Down syndrome. In this case this 17-year-old boy should have been ruled out earlier than the age of 17. I would conduct a cranial nerve examination using Snellen chart test for visual acuity to see if there are any deficits in the optic nerve. Patients near vision could also be assessing (Jarvis, 2016).  An ophthalmoscope consult should be place to “examine the ocular fundus to determine the color, size, and shape of the optic disc” (Jarvis, 2016, p. 644). I will also get order for a CT scan to rule out ICP  if during retina exam  patient’s margins of the optic disc become blurred and indistinct. I would first want to initially suspect that the patient has papilledema, also known as a choked disc which is an increase in cranial pressure (ICP) (Jarvis, 2016).

As a Health care professionals we are educated and trained on strategies to teach patients and family members about this condition. First and foremost, would be for the patient to have genetic testing done to confirm a Down syndrome diagnosed. If the test results are positive, then the health care professional can provide the family and patient with the appropriate resources and specialty professionals to aid them in coping with their new diagnosis. The one most important thing that a health care professional can do is to empower the patient and family. This can be accomplished by carrying out interventions in a manner in which family members acquire a sense of control over their own efforts to meet their needs. Health care professionals can also enable families by creating opportunities for family members to become more competent and self-sustaining with respect to their abilities to mobilize their social networks to get their needs met and attain desired goals (Cohen, 2013). It is important to also let the patient know that his career of playing baseball may be limited in college may grossing diminish if other manifestations of condition sets in later in life.

Some special considerations that can help to educate the patient and family on health promotion and disease intervention would include continuous monitoring measures, including yearly audiologic assessment and yearly ophthalmologic assessment, ongoing management of manifestation of the disorder and related conditions, as well as discussion of issues identified with the move into adulthood. If it were discovered that the patient has developed the habit of using smokeless tobacco, I would educate him on the harmful effects of smokeless tobacco such as the increase risk for cancer, and encourage him to seek out measures to quit.  I would also remind him that at the age of 17 it is illegal to obtain to tobacco products and that he could jeopardize his chances of playing baseball on the collegiate level via an athletic scholarship if caught breaking the law. Furthermore, I would encourage this patient to seek alternative measure to quit smoking, and pick up healthier habits like chewing sugar free gum in place of using smokeless tobacco.

In the Final Project for this course, you are to develop a reflective Ethical Autobiography, intended to define your ethical perspective as an emerging counselor in training.

In order to understand clearly where you are headed, you must also evaluate where you have been and what has influenced you along the way. It is important to reflect critically upon your own values (and sense of personal/professional ethics) and how you developed these perspectives in order to develop an ethical framework. To help accomplish this goal, the Final Project for this course is an Ethical Autobiography in which you will explore various elements of your life experiences that might influence your future ethical framework.

As you reflect on your journey through this class, some of the course readings may have informed your Ethical Autobiography. You can also make use of outside resources, but much of the paper will be exploring what you bring to the profession and events that may have influenced your ethical lens. This reflective autobiography should have personal meaning to you and should help you understand what being an ethical practitioner means. In this sense, you are writing an intellectual and Ethical Autobiography, that is, who you are as virtue of what you believe, what you do, and what you have read. Think broadly—there are no wrong answers; you are exploring your own world.

Vivia’s PostIn the state of Illinois, the American Health Ranking 2017 reported some of the key health indicators, which are the uninsured rates among adults and children; poverty rate, mental illness and substance abuse; prevalence of disease in children and adults; drug death and premature deaths; prevalence of diseases and health risk factors such as diabetes, asthma and obesity. The annual report reviews also reported a low prevalence in mental illness; a decline in excessive drinking; a decline in the uninsured; drug deaths and premature deaths. Significant to these declines, data showed that there is a higher number of primary care physicians and dentist in Illinois. The problems with obesity, however has increased from 17% from 27.1% to now 31.6% in the past five years. Violent crimes have also increased from 15% from 380-436 offences per 100,000 population. Smoking was found to decrease in the past five years; 24% from 20.9 to 15.8% of adults; the uninsured decreased 54% from 14.8% to 6.8% of the population. Another problem is the high level of air pollution, which in part explain the high level of preventable hospitalization.

According to information provided by Medicare and Medicaid. Gov. there are limits to what is covered and what is not covered. People need private insurance and supplements to have sufficient health coverage. Medicare does not cover most dental care, long term care (custodial care); eye examination related to prescribing glasses; dentures, cosmetic surgery, acupuncture; hearing aids and routine foot care. Medicaid, on the other hand covers a broad level of health insurance coverage, which includes doctors’ visits, hospital expenses, nursing homes, home health care and long-term care costs.

Although the Affordable Act is in progress, everyone still cannot afford insurance, which is a direct burden on the health care of individuals. The cost of insurance remains high for many, and obesity, morbidity and premature mortality are some of the direct and indirect burden on health care. Center for Disease Control and Prevention (CDC, 2017) identified, for example, that obesity is a complex issue because it results from a combination of causes and factors such as behavior and genetics. Obesity is a serious burden because it is associated with poor mental health; reduced quality of life and one of the leading cause of death in the USA with other diseases such as diabetes, heart disease, stroke and some types of cancer.

Statistics and estimates of the indicators does provide valuable information for policy makers, which allows them to monitor progress and make decisions about current health risks and problems in the public (Brownson, Chiriqui, and Stamatakis, 2009) This is evident through the US Department of Health and Human Service who initiated a national partnership that involves the federal government, the states, local communities and the private and public sectors to build ‘Healthy People’ 2020, which can impact these indicators (Pender, Murdough, and Parsons,2014).

Florida is a state with a unique population. A large percentage of its population is snow birds and retirees. Some of the strengths of this state include but are not limited to: low air pollution, low obesity rates, and low prevalence of cardiovascular related deaths (United Health Foundation, 2018). Some of the challenges of this state, again, include but are not limited to: a large number of the population being uninsured, an increased prevalence of physical inactivity, and a high level of incidence of salmonella (United Health Foundation, 2018).

I think one of the challenges of this population, not having insurance, has to do with their retirement status as well as a lack of awareness on how to access necessary resources. With Medicare not covering long-term care, hearing aids, dentures, etc. this is likely the root of many of the challenges of this area as well. The aforementioned likely play a predominant role in the lack of physical activity of this population.

Burdens of many of the health risk behaviors and disease are likely cost related. I think by the time people whom are uninsured go in to see a provider, their condition is much more progressed and severe than if they had been insured, feeling as if they could see a provider when the problem initially presented itself. With a lack of long term care coverage I think many people end up living in situations where they are simply getting by instead of thriving.

I think the information found on the America’s Health Rankings site is incredibly important to help guide health policy and the topics that are needing to be addressed.

Careful tracking and record keeping is crucial. In addition, each patient has multiple prescriptions, and one pharmacist fills each order.

  1. Based on the above information, create an entity-relationship diagram that shows the data storage requirements for the system using Visio or its open source alternative software. There are several points in the order-fulfillment process at which information must be recorded in the system. Case manifests must be printed at the start of each shift. Because a prescription may take a long time to be used completely, information must be entered into the system each time a medication is sent, noting the quantity sent and which pharmacist filled the prescription for that shift.

Prescriptions contain basic information such as ID number, original date of order, drug, unit of dosage (pill, teaspoon, suppository), size of dosage, frequency of dosage, and special instructions (take with food, take before meals). There are several types of prescriptions, each with unique characteristics. Some orders are for a single, one-time-use prescription. Some orders are for a number of dosages. Some orders are for a certain time period. Information about the prescription order must be maintained. An order occurs when the nursing home phones in the needed prescriptions. Because prescriptions may last for an extended period of time, a prescription is a separate entity from the order itself. The system records which employee accepted and entered the original order.

The system has basic data about all drugs. Each drug has generic information such as name, chemical, and manufacturer. More detailed information for each type of dosage, such as the size of the pill, is also kept. A single drug may have many different sizes and types.

Information about the fulfillment of orders must also be maintained. For example, on a prescription for a number of pills, the system must keep a record of each time a pill or number of pills is dispensed. A record is maintained of which pharmacist fulfilled the order. Assume all prescriptions are dispensed only as needed for a 12-hour shift.

  • Public health surveillance is one way that public health officials target intervention strategies (Turlock, 2016). Often, it is through prompt recognition of and reporting of incidents of communicable disease that a disaster can be averted (Turlock, 2016). Surveillance activities often prompt questions such as, What is causing the disease? How is it spreading? And who is at risk (Turlock, 2016)? While it is true that preparedness planning cannot eliminate all traces of threat to a community, planning assures that medical services and treatment are deployed in an effective, efficient, and rapid manner (Turlock, 2016). Public health plays a vital role in coordination of providers, assurance of supplies particularly when the Strategic National Stockpile pharmaceuticals and supplies are required, and mobilization of state and national response systems. Public health officials may also provide health care services when required (Turlock, 2016).
  • Stanhope (2016) noted that evidence-based practice (EBP) has become more important in health care for many reasons: increased expectations of consumers, increased availability of information through the Internet, increased accountability for results, health care economic changes, and growing numbers of lawsuits, among other reasons. EBP is a lifelong problem-solving approach that regularly produces excellent results and often provides the theoretical underpinnings for programs to mitigate problems in the community. Once programs are in place, evaluation of their effectiveness should be conducted to determine whether they are worth the continued expenditure of resources. Use of EBP is vital to assure safe outcomes for populations during disasters, such as massive communicable disease outbreaks, and should be the foundation of disaster-planning strategies.
