CLIENT 3 CASE

Client is a 22 year old female. She states she has come to counseling because she wants to do a better job knowing herself and be brave enough to let others know her too. Client reports that she came out as bisexual several months ago and has had mixed reactions to this announcement. She states that she has told a few friends and some of them have disowned her. She also states that she has not told her parents or her best friend because she is worried that the same thing might happen (being disowned). She reports being conflicted about her sexuality for the past 4 or 5 years and states that she has known that she was different for a long time. She reports that she did not do anything about this in high school and ignored how she felt because she was worried about what others would say. She states now that she is in college and on her own that she feels it is safer to be able to explore her sexuality. She reports three relationships with men and two relationships with women but the relationships with women were a secret. She states that the last woman broke up with her because the client insisted on secrecy.

Client reports a stable family environment growing up in a two-parent home. She reports that her mother was very controlling and tried to make her do ‘all sorts of things’ while she was growing up. She states she mostly complied because she did not want to get into trouble. She reports that her dad was somewhat dismissive and allowed her mom to ‘control the house’. She states that she received good grades throughout high school and was basically ‘a stellar child’. She reports that she played basketball and tennis and was interested in sports her junior and senior year. She reports one older brother and one younger sister and strong relationships with both siblings. She reports that she has lived away from home for the past three years with the first year on campus and the last two years living in a house with friends. She has been part of a sorority since her freshman year and feels very connected to the women who also are part of the sorority. She is worried that if they find out about her secret, they might kick her out of the sorority. Client states that she is studying environmental sciences and is happy with this choice.

Client reports little income and is primarily supported by her parents. She states she works part-time on campus in the dean’s office and she likes this job, but she knows she needs to find something different that is more in her field of study. She reports no drug use, but she does reports some alcohol use. She states she primarily drinks when she is at parties with her sorority sisters. She reports no past or current legal problems. She reports no medical problems. She states that she has grown up Catholic, but she is currently not practicing. She states that she believes there is a God but she does not know how to reconcile this belief with her thoughts and feelings about herself. She states that her faith only makes her feel guilt. She states that she is worried that she is bisexual and that she is never going to ‘pick a side’. Her gay and lesbian friends are always joking around with her because she just ‘falls in the middle’. She reports this is her only real problem of concern. Her grades are As and Bs and she is on track to graduate in the fall.

Students will choose one of five assigned constructs and focus their handbook on this specific area of learning and cognition. Skills and information learned throughout the course will be applied in the design and creation of this handbook. Findings from required sources, including those from the text and individual peer-reviewed articles, will be incorporated into the handbook; however, these findings will not constitute the total information for each of the sub-constructs addressed within the project. Students should include the relevant sources they researched in the Week Two Discipline-Based Literature Review as well as those from the Week Three Assignment. The purpose of the handbook is to share helpful strategies, apply the chosen construct to seven sub-constructs in the field, and present a holistic guide for others that can be used in the discipline.

I have worked as RN in a level one trauma center for about 3 years now, working in the Burn ICU has been one of the biggest challenges I have faced in my nursing career. Some people may assume that the Burn ICU does not care for very sick patients like other units, but in actuality it is so specialized that a well-trained CTICU nurse would struggle. One of the biggest learning experiences I faced since being in the Burn ICU was during a horrible burn trauma activation involving 6 male individuals that were burned while working at the local power plant. It was about 5:45pm, when the charge nurse pager went off with a burn trauma alert. Yes, I happened to have taken over charge nurse at 3:00pm, while managing one patient in the ICU. Back to back the pager went off 6 times, with different burn trauma alerts. Before I knew it, my charge phone was ringing and it was the hospital supervisor informing me that we were going to be transferring out all the patients in our 6 bed ICU to other ICU’s to make room for the 6 burn victims arriving. Suddenly, before we knew it we moved all of our patients and the new burn victims came rolling in. All of the burns were ranging from 60-90% total body surface. I individually managed one of the patients while navigating the unit and med-surg burn floor. It was about 6:30pm, I called in 4 nurses to come in early to help assist with debriding these patients. By the end of the shift at 7pm, all 6 patients were being lined and started on CRRT, along with arterial lines placed and central lines. I somehow had to manage my nursing skills appropriately and take charge of the entire unit. At 7:15p, I lost pulses on my all 4 extremities and my patients bladder pressure was sky rocketing. After about 10 minutes, the plastic surgery team was at the bedside performing fasciotomies and trauma surgery was opening up my patients’ abdomen at beside to help relieve bladder pressure.

During this one individual experience I learned so much about myself. My time management, assessment skills, nursing knowledge and team leadership skills were all put to a test. Thinking back I thought I would have fallen apart, I had only been a RN for a little under two years at this time and I was still questioning all of my skills and abilities. Walking out of the hospital that night I could have not been any more exhausted and discouraged, wondering if I had messed something up or forgotten something. I tried my best to think to myself “you are leaving and you kept this 90% full thickness burn victim alive, that is what you are here for.”

Breastfeeding is the most essential, dynamic and profitable way of providing an infant with the nutrients they need as they enter the world outside of the womb (Farhan, Das & Malik, 2018).

Not only does breastfeeding benefit the infant, but it has important consequences for the mother as well. Breastfeeding can lower the risk for certain cancers including breast and ovarian as well as decrease the risk of developing type 2 diabetes (Grummer-Strawn et al., 2013). The CDC has started many initiatives to ensure that mothers in the United states have adequate access to lactation services and understand the best way to nourish their newborn. That being said, there are still many mothers who are unable to breastfeed but there is hope for those who desire to have breastmilk for their baby. Donor breastmilk is the milk of choice when a mother’s own milk is not available. Research clearly demonstrates that human milk decreases the rates of pediatric infections such as necrotizing Enterocolitis (NEC) and sepsis (Coutsoudis et al., 2011).

Encouraging mothers to research the benefits of breastfeeding and educating as the mother and family as they are willing is the goal of the nurse practitioner. Ultimately, it is the choice of the mother to partake in breastfeeding, but proper education and encouragement should be offered for new parents.

The CDC is currently on a mission to improve the breastfeeding in the nation by current, new, and prospective mothers. Nowadays the CDC is actively promoting and engaging with new and potential mothers and well as past mothers to gather information regarding their pregnancies, determining thoughts and practices and how to move forward with the promotion of breastfeeding (CDC, 2018).  From years 2011 to 2015, the CDC has been conducting a study to determine the difference in breastfeeding by demographic variables, which provided them with information regarding cultures and areas where improvement could be made, and where their advocacy needed to be expanded and promoted.  These areas have gained additional promotion regarding health benefits for both mom and baby, as well as the advantage of a cheaper alternative to formula (Anstey et al., 2017).  A variety of issues with moms were discovered in these areas concerning breastfeeding and its practice. Some of these new moms are unsure of breastfeeding and unfamiliar with the method. Some of the moms are interested in breastfeeding but then revert to formula.

The CDC has adopted the ten breastfeeding steps that are being successfully implemented by the WHO and UNICEF organizations; and has been used for promotion throughout hospitals and a variety of healthcare settings as a tool to promote and to make breastfeeding a successful and healthy experience for mom and baby (Sriraman, 2017).  This promotion could be critical as many mothers would attempt to breastfeed their baby as well. This guide and assistance gives new moms information and a guideline to help their newborns attach.  They could also meet with a support team that assists them and has the specialized experience to encourage and teach the mom with the proper practice of breastfeeding.

As practitioner one should encourage a mother to breastfeed if possible, because of the significant benefits of it, such as the decrease chance of the baby of having an allergy to the mother’s breast milk.  However, there could be contraindications to breastfeeding that must be looked at as well.  According to the CDC (2018), a mother who is addicted or who has a disease should not breastfeed, such as a mother with HIV because the disease or drug could pass to the baby through the breast milk.
